The "Advanced Form" allows greater customization and building of custom controls. These controls include simple text boxes, yes/no fields, checkbox groups, radio button groups and drop down lists. Fields can be set as required or optional and can be grouped into sections. If you desire, feedback submitted through the advanced form can be stored in a database for later download.
